About the Role:
Social Worker (Integrated Triage & Assessment) Grade 10
Ceredigion County Council is seeking experienced agency social workers to join our Integrated Triage and Assessment Service (ITAS) – Adult Service, a pivotal front‑door service at the heart of adult social care decision‑making.
ITAS Adults offers a fast‑paced, professionally rewarding environment where social workers are supported to undertake high‑quality proportionate assessments, provide clear advice and assistance at first contact, and help ensure people receive the right support at the right time. The service plays a crucial role in reducing escalation, supporting hospital discharge flow, and promoting timely access to community‑based support.
Working within ITAS Adults provides:
- Exposure to diverse and complex practice, including safeguarding, mental health, self‑neglect and hoarding.
- Strong multi‑agency working with health, housing and third‑sector partners.
- Clear management oversight, regular professional support, and a team‑based approach to decision‑making.
- A focus on short‑term, purposeful involvement, supporting clarity, progression and appropriate step‑down.
The service is based in Aberaeron and operates within a supportive, collaborative culture where agency workers are valued as integral members of the team.
