About the Role:

We are seeking a dedicated Adults Social Worker to join our dynamic Locality Team. This is an exciting opportunity for a compassionate and skilled professional to deliver high-quality social care services that empower adults to live independently and safely within their communities. 

As an Adults Social Worker, you will play a key role in assessing needs, managing caseloads, and developing person-centred care plans in line with the Care Act 2014 and safeguarding responsibilities. 

Key Responsibilities: 

• Conduct strengths-based assessments for adults with a range of needs including physical disabilities, mental health conditions, and learning disabilities. 
• Manage a varied caseload within a locality-based social work team. 
• Develop, implement, and review person-centred support plans. 
• Carry out safeguarding investigations and ensure compliance with statutory duties. 
• Work collaboratively with health professionals, families, carers, and partner agencies. 
• Facilitate access to services that promote independence, wellbeing, and choice. 
• Maintain accurate and up-to-date case records and reports. 
• Support hospital discharge processes and community transitions. 

Requirements: 

• Qualified Social Worker (BA/MA Social Work or equivalent). 
• Registered with Social Work England. 
• Proven experience within Adult Social Care or Local Authority settings. 
• Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding frameworks. 
• Excellent assessment, communication, and decision-making skills. 
• Ability to work autonomously and as part of a multi-disciplinary team. 
• Full UK driving licence (desirable depending on locality). 

Desirable Skills & Experience 

• Experience working in a locality or community-based team. 
• Knowledge of Deprivation of Liberty Safeguards (DoLS). 
• Experience with hospital discharge planning or reablement services. 
• Familiarity with multi-agency working and integrated care systems.
