About the Role:
We are seeking a dedicated and compassionate Adults Mental Health Senior Practitioner to join our Adult Social Care team. You will support adults experiencing complex mental health challenges, promoting independence, wellbeing, and recovery through person-centred care planning. You will work in line with the Care Act, Mental Health Act, and safeguarding responsibilities.
Key Responsibilities:
- Conduct comprehensive mental health assessments and risk evaluations
- Develop and implement person-centred care plans
- Work collaboratively with community mental health teams
- Safeguard vulnerable adults and manage risks
- Apply legislation including MHA, MCA, and Care Act
- Coordinate access to housing, employment, and support services
- Maintain accurate case records
- Advocate for service users and promote independence
- Participate in multi-agency meetings
- Provide crisis intervention where required
Requirements & Qualifications:
- Degree in Social Work (BA/MA or equivalent)
- Registered with Social Work England/Social Care Wales
- Experience in adult mental health or social care
- Knowledge of UK mental health legislation
- Strong communication and report-writing skills
- Ability to manage complex caseloads
