About the Role:
We are seeking an experienced Adults Team Manager to lead a dynamic multidisciplinary team supporting safe and timely hospital discharge. This is an excellent opportunity for a qualified social work professional with strong leadership skills to drive high-quality, person-centred care and improve outcomes for adults transitioning from hospital to community settings.
Key Responsibilities:
- Lead and manage a team of Adult Social Workers, Social Care Practitioners and Discharge Coordinators
- Oversee the delivery of safe, efficient hospital discharges in line with Discharge to Assess (D2A)
- Ensure compliance with Care Act assessments and safeguarding procedures
- Promote person-centred care planning
- Manage complex cases involving frailty, dementia, and physical disabilities
- Work with NHS teams and community services
- Monitor team performance and caseload allocation
- Support staff development through supervision
- Lead safeguarding enquiries
Essential Requirements:
- Qualified Social Worker – Social Work England registered
- Experience in adult social care or hospital discharge
- Experience in a Team Manager or Senior role
- Knowledge of Care Act, MCA, and safeguarding
- Strong leadership and communication skills
Desirable Skills:
- NHS or integrated care experience
- Knowledge of Continuing Healthcare
- Experience leading service improvements
