About the Role: 

We are looking for an experienced Adults Hospital Discharge Social Worker to join a dynamic Adult Social Care team. This role focuses on supporting safe, timely, and effective hospital discharges, ensuring individuals receive the right care and support in the community. You will work in a fast-paced hospital setting, collaborating with multidisciplinary teams to deliver person-centred outcomes and reduce delayed transfers of care


Key Responsibilities 

  • Assess patients within hospital settings to support safe and timely discharge planning. 
  • Undertake Care Act assessments and develop person-centred care and support plans. 
  • Work collaboratively with NHS professionals, discharge coordinators, families, and external agencies. 
  • Ensure compliance with Care Act, Mental Capacity Act, and safeguarding duties. 
  • Coordinate community services, placements, reablement, and support packages. 
  • Identify and manage risks, ensuring safeguarding concerns are addressed promptly. 
  • Maintain accurate, up-to-date case records in line with statutory requirements. 
  • Contribute to reducing delayed discharges and improving patient outcomes. 


Requirements: 

  • Qualified Social Worker (BA/MA Social Work, DipSW or equivalent). 
  • Registered with Social Work England/Social Care Wales 
  • Experience in Adult Social Care, ideally within a hospital discharge setting. 
  • Strong knowledge of legislation including the Care Act and Mental Capacity Act 
  • Excellent assessment, decision-making, and organisational skills. 
  • Ability to work under pressure in a fast-paced hospital environment. 
  • Strong communication and multi-agency collaboration skills. 


Desirable Skills: 

  • Previous experience in discharge to assess models. 
  • Experience working with complex cases including dementia or long-term conditions. 
  • Knowledge of Continuing Healthcare (CHC) processes. 
  • Experience working within integrated health and social care teams.
