About the Role:
We are seeking a dedicated Adults Community Social Worker to support vulnerable adults, promote independence, and ensure safeguarding and wellbeing in a community setting.
Key Responsibilities:
- Conduct assessments under relevant legislation
- Develop person-centred care plans
- Work with multi-disciplinary teams
- Safeguard vulnerable adults
- Maintain accurate records
- Promote independence and wellbeing
Requirements:
- Degree in Social Work
- Registered with Social Work England
- Experience in adult social care
- Strong safeguarding knowledge
